This name will be used on all race documents​, e.g. schedule, brochure and team event uniform. The name should be no more than 40 characters including spaces.

Students (aged 16-18 Years 12 & 13) must provide parental consent forms for all student participants.

I, as the above named applicant / appointed crew manager for my team, confirm that I have received and understood the CDBF terms and conditions and have signed below to agree to be bound by the said terms once the application is accepted.
